Sophie and Natalie discuss where familiar CBT models may fall short and how earlier experiences, shame and cycles of anxiety can shape a person’s current fears.
They explore mapping the formulation collaboratively, working with avoidance and safety behaviours, and bringing GPs and health teams into the conversation.
Familiar CBT cycles can be useful, but Dr Sophie Gwinnett explores what may be missed when the formulation focuses only on current thoughts, checking and reassurance seeking. Earlier experiences, shame and the meaning attached to physical sensations may also shape the person's fears.
The “house of health anxiety” offers a layered way to organise that information. It can help therapist and client understand how current anxiety and safety behaviours connect with a wider history, without reducing the person to a list of symptoms.
Building the formulation together makes it something the client can recognise and use. Sophie and Natalie discuss mapping avoidance and safety behaviours, then considering which layer of the problem an intervention is intended to address.
The episode also considers collaboration with GPs and health teams. This matters because health anxiety sits alongside real bodies and real healthcare experiences; therapy needs to hold the psychological formulation without treating every physical concern as automatically explained by anxiety.
